Dr. Michael Buxbaum
Board Certified Family Practice
Dr. Michael Buxbaum is a Board-Certified Family Practice Physician with an office and facility practice in the greater Houston area. He is a Clinical Assistant Professor in the Department of Family and Community Medicine at the Baylor College of Medicine and is affiliated with the Memorial Hermann Physician Network serving as an Advanced Primary Care Physician. Dr. Buxbaum was recognized on Achievement of Recognition for Delivering High-Quality, Patient-Centered Care in 2024 by the National Committee for Quality Assurance. Dr. Buxbaum is the Chief Wellness Director for Retirement Center Management, a company that manages multiple senior living communities in Texas. He serves as Medical Director for The Courtyards of Pasadena Skilled Nursing Facility. Dr. Buxbaum is a Cum Laude honors graduate of The University of Texas at Austin. After graduating from The University of North Texas Health Science Center in Fort Worth in 1998, Dr. Buxbaum completed a Family Practice residency at St. Joseph Hospital in Houston in 2001.
Dr. Pippa Schnee
Dr. Schnee focuses her practice on home visits in assisted living facilities. She completed a Bachelor of the Arts at Trinity University with a major in Philosophy. Prior to medical school, she worked on joint research projects between Texas Heart Institute and University of Texas Health Science Center focusing on subcellular changes in heart tissue before and after implantation of LVAD (left ventricular assist device, or “artificial heart.”) She has publications stemming from that research in Journal of Cardiac Surgery, Journal of Heart and Lung Transplantation, and Medscape. Dr. Schnee then discovered her love of family medicine at University of Texas Health Science Center at Houston (now McGovern Medical School) where she received her MD. She matched for internship and residency in Family Medicine at Duke University. During residency ,she was able to combine her interest in philosophy with her love of medicine by serving on the Duke GME Ethics Committee. She also discovered her love of teaching, taking many opportunities to teach aspects of medicine to medical students, physician assistants, high school students, and members of the community. These efforts led to her winning teaching awards from both medical students and faculty at Duke. After completing internship and residency at Duke, she became an attending physician at Duke Primary Care. She concurrently held a Consulting Associate faculty appointment with Duke University Medical School, and was invited to teach the Duke Family Medicine Residents as a guest lecturer. Her family then relocated to Houston, where Dr. Schnee found a new area of focus for her loves of medicine and teaching in joining Dr. Buxbaum. Dr. Schnee brings her love of and talent for teaching into her relationship with patients and families, helping them understand their health issues and what their treatment choices are.
Megan Shonebarger PA
Megan attended Texas A&M University in College Station where she received a Bachelor of Science degree in psychology with honors. She then attended University of Texas Medical Branch in Galveston where she received a Master of Physician Assistant Studies with honors. She has been practicing as a PA since 2013, and has prior experience in endocrinology and gastroenterology. She now enjoys primary care with a focus on rounding at assisted living facilities.
Tiffani Arriazola NP
Tiffani is an Adult Gerontology nurse practitioner who specializes in young adults, adults, older adults, and the elderly at a variety of skilled nursing and assisted living facilities. Her background of work includes; emergency nursing, geriatrics, and medical surgical. She completed her Bachelor of Science in Nursing at The University of Texas Medical Branch of Galveston and then earned her Master of Science in Nursing at the University of Texas Medical Branch of Galveston in 2017. She holds a board certification with the American Academy of Nurse Practitioners as an Adult Gerontology Primary Care Nurse Practitioner. She is also a member of the American Association of Nurse Practitioners.
Nicole Fawcett NP
Nicole is an adult geriatric nurse practitioner that provides primary care to adolescents, adults, and geriatrics. She strives to provide holistic, evidence-based care to her patients and especially enjoys working with patients in skilled nursing and assisted living facilities. Nicole received her Bachelor of Science in Nursing (BSN) at The University of Texas Health Science Center and her Master of Science in Nursing (MSN) at The University of Texas Medical Branch. She is certified through the American Academy for Nurse Practitioners (AANP).
