Dr. Michael Buxbaum

Dr. Buxbaum founded the practice with a vision of bringing consistent, high-quality primary care to patients in every setting — from the medical office to assisted living communities, nursing homes, and memory care facilities. Board certified in family medicine, he brings over 20 years of experience in caring for patients of all ages, with particular expertise in geriatric medicine and chronic disease management. He believes that patients deserve the same quality of care regardless of where they live, and has built a practice that delivers on that promise across more than 15 communities in the Houston area.

Dr. Pippa Schnee
M.D.Board Certified Family Medicine
Dr. Pippa Schnee is a board-certified family medicine physician with a passion for improving the lives of older adults. Since 2015, she has focused on providing medical care in assisted living and skilled nursing facilities. She treats chronic illnesses—especially heart-related conditions—to help patients avoid hospital stays and maintain a strong quality of life. Dr. Schnee earned her medical degree from McGovern Medical School in Houston (formerly UT Houston Medical School) and completed her residency at Duke University Hospital, where she received awards for excellence in teaching. She later worked at Duke Primary Care and served as a Consulting Associate, training medical students and residents. Before medical school, Dr. Schnee conducted cardiology research with the Texas Heart Institute and UT Health Science Center, with publications in the Journal of Cardiac Surgery, Journal of Heart and Lung Transplantation, and Medscape. Known for her thoughtful communication and patient-centered approach, Dr. Schnee works closely with patients and their families to ensure care decisions reflect their goals, values, and long-term needs.

Megan Shonebarger
PA-CPhysician Assistant
Megan is a certified physician assistant who brings a warm, patient-centered approach to every visit. She works closely with our physicians to ensure continuity of care across all settings, with a special focus on nursing home and assisted living patients. Her ability to build trusting relationships with patients and facility staff makes her an essential part of the Buxbaum Medical team.

Nicole Fawcett
NPAdult-Geriatric Nurse Practitioner
Nicole is an Adult-Geriatric Nurse Practitioner dedicated to delivering comprehensive primary care to adolescents, adults, and older adults. She is passionate about providing holistic, evidence-based care tailored to the unique needs of each patient, and she especially enjoys caring for residents in skilled nursing and assisted living facilities. Nicole earned her Bachelor of Science in Nursing (BSN) from The University of Texas Health Science Center and her Master of Science in Nursing (MSN) from The University of Texas Medical Branch. She is nationally certified through the American Academy of Nurse Practitioners (AANP).
